Appointments are available with the practice nurse, normally by GP referral, but you can make an appointment directly at reception, if you feel that it is necessary.

Appointments are available with the practice nurses. In addition we send invitations to patients who are due for a cervical smear following recommended guidelines from the National Cervical Screening Programme. The general recommendation is:

Undertaken in the autumn, the flu clinic is widely advertised around the surgery, on prescriptions and under 'Further Information'. Please make an appointment with the Practice Nurse

A surgery is held for removal of abnormal benign skin lesions and joint injections. Referral is by consultation with your Doctor
An excellent palliative care service is provided by the Community Nursing Team in partnership with their colleagues from the specialist services. Available by GP referral.

You may need travel vaccinations and health advice, depending on where you are visiting. To help us advise you on the protection you may need. Please complete the Travel Health Questionnaire below and bring this with you to your appointment. We advise you to book your appointment at least 8 weeks before departure date.
Not all travel vaccinations are included in the services provided by the NHS. Please see the travel vaccine charges below. You will be asked to pay for the vaccines you have received on the day of the appointment.

You do not require a doctor's certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or at our reception or on the HMRC website or they may provide you with their own form for completion
If you have been off work sick for more than seven days you will need to get a Statement of Fitness to Work (fit note) from your GP or the doctor that treated you in hospital.
The fit note allows your doctor to provide you with more information on how your condition affects your ability to work. It may suggest ways in which you can return to work, for example, changes to your working hours or different duties for a temporary time. This will help your employer to understand how they might be able to help you return to work sooner. You and your employer will be able to talk about how this will benefit your return to work.
The Fit Note replaces the sick note. It is the new form that your doctor will give you when your health affects your ability to work. More information can be found about understanding Fit Notes from www.direct.gov.uk/en/Employment/Employees/Sicknessabsence/
IMPORTANT NOTE: IF YOU NEED A CONTINUATION OF A SICK NOTE - YOU MUST ALLOW AT LEAST 7 DAYS FOR THIS TO BE PROCESSED. CONTINUING SICK NOTES WILL NOT BE ISSUED ON THE SAME DAY AND THE RECEPTIONISTS WILL NOT BOOK URGENT APPOINTMENTS FOR THIS PURPOSE.
WE REGRET THAT WE WILL NOT ORDINARILY ISSUE DUPLICATE SICK NOTES - IF YOU ARE HANDING YOUR SICK NOTE TO ANOTHER AGENCY OR IN CASE OF LOSS, WE RECOMMEND THAT YOU TAKE A PHOTOCOPY OF IT BEFORE DOING SO.

If you look after someone, perhaps a friend or a relative, who is frail, has an illness or a disability which means they can't manage at home without your care, you may be entitled to help.
Some carers and the people they care for are entitled to benefits. There are also many other ways you can get help, information and support.
You can register as a Carer here at the practice by completing a Carers Identification and Referral Form (collect one from the practice or download one from the link below).
