New Patient Registration

HOW TO REGISTER WITH THE PRACTICE

 

If you would like to register with our Practice and are entitled to NHS treatment, then please come along to the surgery where the procedure for registering is as follows:-

 

  • Production of your NHS Medical Card
  • Photo Identity (such as passport, driving licence etc) and proof of residency (such as recent utility bill, official letter etc)
  • Be resident within our defined catchment area, please see the map at the back of the booklet.
  • If you do not have a medical card, please come along with the other documentation stated. You will be asked to complete a New Patient Questionnaire and the information will be sent to the Dorset Family Health Services Agency so that a medical card can be issued to you. You are most welcome to make an appointment at the time or registering if you would like to, but this is not mandatory. The Doctors in the Practice would recommend however, that all newly registered patients partcipate in a consultation within 6 months of joining the Practice
